Provincial/Territorial Committee (P/TC) Request for Certificate of Insurance
Certificates must be requested by the P/TC Chair
Certificates of Insurance may be issued where proof of insurance is requested by a landlord or Association that is permitting the cadets or the League to operate on their property. These certificates are “event specific” to League activities as authorized and approved by the League but can sometimes be issued for “Block” proof of coverage, for example for use of a facility on a weekly basis during an extended period.
Important things to know when filling out this electronic request form:
Prior to filling out this form, make sure that you have all the information needed from the landlord, corporation or agency you are dealing with. This includes their deadline to receive proof of insurance, their contact information, street address, email address, phone number, if they require to be added as an additional insured, etc.
The certificates are immediately forwarded by e-mail to all the parties listed on the request form. If you are unable to get an email address, it will be your responsibility to provide the certificate of insurance to them
Following our annual Insurance policy renewal process, certificates can only be requested for the CURRENT TRAINING YEAR (not exceeding June 30th annually). This certificate of insurance request process should resume by mid-July for any proof of insurance required for the upcoming training year.
In the case of any error, you will need to submit the request again.

An additional insured is an entity/company's legal name, that is added to the insurance certificate. This then provides coverage under that policy during the course of the event and or rental of premises. Although not all companies you deal with will require this, it is important to be clear on if they wish and need to be added. The question should be asked at all times. If "yes" you need to ensure and obtain the proper and full legal name of the company along with the complete mailing address to include it in this section.
